Turn a clinical note into HPO codes.

PhenoLens reads a clinical note and codes every finding in the Human Phenotype Ontology (HPO). Each phrase is highlighted, marked present, absent or history, and anything it could not code is flagged so nothing is missed.

HOW IT WORKS

From note to codes in four steps

01

Paste or dictate

Paste a note or dictate one with your browser's speech service. Leave out anything that identifies the patient.

02

Find the findings

PhenoLens picks out each clinical finding and whether it is present, absent (“no rash”) or past history.

03

Code each one in HPO

Each phrase is matched to an HPO term: an exact name or synonym first, then a shortlist ranked by meaning, then an AI model picks the best fit.

04

Review and export

Check the highlighted phrases, see what could not be coded, and export the codes as IDs, a list or a GA4GH Phenopacket.

What is an HPO code?

The Human Phenotype Ontology is a standard vocabulary of clinical findings used in rare disease and genetic diagnosis. Each finding has an ID. For example, “fever” is HP:0001945 and “joint pain” is HP:0002829 (arthralgia). Genetic testing labs and diagnostic software such as GENA 3.0 use HPO codes to compare a patient against known disease profiles.

Why absent findings matter

A note that says “no rash” rules diagnoses out. PhenoLens codes it as an absent finding instead of dropping it or, worse, coding it as present.

Privacy

Notes are sent to an AI model to find and code the findings, and are not stored. Note text is never logged. If you report a wrong code, only that phrase (up to 120 characters) and the codes are kept. Do not enter identifiable patient information.

PhenoLens is a free research preview by Sivotec Inc. It is not for diagnosis or treatment, and its output may be wrong, so verify every code.

FAQ

Common questions

Is PhenoLens free?

Yes. PhenoLens is free to use in your browser and needs no account.

What can I export?

HPO IDs, a readable list of findings, or a GA4GH Phenopacket for use in other tools.

Does PhenoLens store my notes?

No. Notes are processed by an AI model to produce codes and are not stored, and note text is never logged. Still, do not enter identifiable patient information.

How accurate is it?

PhenoLens matches exact HPO names and synonyms first, then uses a ranked shortlist and an AI model for harder phrases. It can still be wrong, so review every code before using it.

What do I do with the HPO codes?

Use them in a genetic test request, a research dataset, or a diagnostic tool. GENA 3.0 turns a patient's findings into a ranked differential diagnosis.
